5.2.2 Lid Eversion

PURPOSE To allow observation of the palpebral conjunctiva of the per lid.




INDICATIONS
Indicated for all contact lens patients or prior to fitting a patient with contact lenses to establish a baseline.Indicated if the patient has a red eye or if the patient’s symptoms,history,or other exam findings,suggest a foreign body under the per lid.




ADDITIONAL EQUIPMENT
·Sterile cotton swab.




STEP-BY-STEP PROCEDURE
1.With the patient positioned in the slit lamp,instruct the patient to look down.
2.Grasp the patient’s per lid at the lid margin between your thumb and index finger.Gently pull the lid down and away from the globe.
3.With your free hand, insert the cotton tip of the swab at the posterior margin of the tarsal plate in the center of the lid.An alternate technique is to use the index or middle finger of your free hand in place of the swab.
4.Gently press down on the swab while pulling the lid ward.Hold the lid margin firmly against the serior orbital rim.
5.Remove the cotton swab and reposition the slit lamp to view the serior palpebral conjunctiva.The illumination arm should be set approximately 30°from the straight ahead position and the beam should be adjusted to a wide parallelepiped.
6.Scan the serior palpebral conjunctiva, looking for elevations, depressions,foreign bodies,or injection.




RECORDING
·The results for this procedure are recorded in the slit lamp recording section under “palpebral conjunctiva.”
·Record for each eye separately.
·There is no indicate that lid eversion was done.By recording observations for the serior palpebral conjunctiva, it is assumed that you everted the patient’s lid.
